Peptides are everywhere — podcasts, gym floors, gray-market websites. Some of the science is real, some is marketing, and the difference matters for your health. Here's the honest map.

What peptides actually are

Peptides are short chains of amino acids — small proteins, essentially — that act as signaling molecules in the body. Insulin is a peptide. So are many of the hormones regulating appetite, growth, sleep and tissue repair. Peptide therapeutics are lab-made versions of these signals, designed to nudge specific biological pathways.

That breadth is why the category feels confusing: 'peptides' spans FDA-approved medications with deep clinical evidence, investigational compounds with early data, and gray-market vials with none at all. Treating them as one category is the first mistake.

The categories, honestly ranked by evidence

A useful way to sort the space:

  • FDA-approved, strong evidence: GLP-1 receptor agonists (semaglutide, tirzepatide) for weight management and metabolic health; insulin; and several clinical peptides. Large trials, known safety profiles, real prescribing pathways.
  • Prescription-adjacent, moderate evidence: compounds like tesamorelin (approved for specific indications) and secretagogues such as CJC-1295/ipamorelin — some human data, prescribed off-label by some clinics, with legitimate debate about long-term effects.
  • Popular but thin evidence: BPC-157, TB-500 and most 'healing' peptides — promising animal studies, very limited human trials. Widely used, honestly unproven.
  • Gray-market 'research chemicals': anything bought without a prescription from unregulated websites. Purity, dosing and even identity of the compound are unverified. This is where most of the horror stories come from.

GLP-1s specifically

GLP-1 receptor agonists deserve their own paragraph because the evidence is genuinely strong: 15–20%+ average body-weight reduction in trials, with meaningful improvements in blood sugar, blood pressure and cardiovascular outcomes. For people with obesity or metabolic disease, they are the most effective non-surgical tool medicine has produced.

They are also not a shortcut around training. A meaningful share of GLP-1 weight loss can come from lean mass if you don't strength train and eat adequate protein while on them. The best results we see pair the medication with exactly what a gym provides: progressive strength work, protein targets and habit coaching. Medication changes appetite; training decides what the lost weight is made of.

Skepticism and curiosity can coexist. GLP-1s are the real deal for the right person under medical care. Some peptides may prove useful as evidence matures; many will not. Nothing from an unregulated website is worth the risk. And no compound replaces the fundamentals — strength training, protein, sleep and consistency — that determine whether any of it produces health rather than just a smaller number on a scale.
